PAY AND BENEFITS

Mental Health Service Coordinator - Salary Range:

Annual Pay Range: $73,699.88 - $93,581.75

Hourly Pay Range: $35.432637 - $44.991225

Bilingual Spanish/English skills are required for this position. An additional 5% of base compensation is added to pay

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

Health Centers-Behavioral Health provides licensure supervision to all regular status employees who have completed a Graduate degree in a behavioral health related field (social work, counseling, marriage and family therapy, etc.) and are actively pursuing a licensure in that field.

Group supervision is available for CADC hours for those on a CADC track.

Clackamas County's Health Centers are Federally Qualified Health Centers.

There may be options for loan forgiveness while working with a government / public agency through programs such as the National Health Service Corps (NHSC) Loan Repayment Program or the Public Services Loan Forgiveness (PSLF) program.

JOB DETAILS AND QUALIFICATIONS

Clackamas Health Centers is seeking a dedicated and confident Qualified Mental Health Associate to join our Behavioral Health team as a Bilingual (Spanish/English) Panel Navigator (classified as a Mental Health Service Coordinator) to support the SHIFT initiative.

The selected Panel Navigator will partner closely with CareOregon's Strategic Healthcare Investment for Transformation (SHIFT) initiative - a statewide effort focused on transforming specialty behavioral health organizations so that individuals with behavioral health needs remain at the center of care and so care teams can thrive. Through a collaborative approach, SHIFT builds client-driven, outcomes-focused, team-based care models that eliminate health inequities and ensure timely access to services.

The Bilingual Panel Navigator supports the SHIFT initiative by coordinating care and facilitating communication among clients, clinical providers, and interdisciplinary care teams. This role leads daily huddle meetings, oversees clinical documentation and scheduling needs, and ensures clients have timely access to specialty services, referrals, and higher levels of care when needed. The Panel Navigator serves as a key point of contact for clients and families, provides outreach and engagement support, and contributes to high-quality, culturally responsive behavioral health care. This position includes direct client contact and plays a key role in improving behavioral health access for Spanish-speaking community members.

Required Minimum Qualifications/ Transferrable Skills:*

• Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) at the time of hire

A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:

(a) Bachelor's degree in a behavioral sciences field; or

(b) A combination of at least three years relevant work, education, training or experience.

• A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position.

• Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection)

• Must possess and maintain a Basic Life Support (BLS) certificate

• Experience working in behavioral health, integrated care, complex case management, and/or crisis services environment

• Experience providing services and/or support to vulnerable clients

• Experience working collaboratively with community partners, collateral service providers, law enforcement agencies and co-workers

Preferred Special Qualifications/ Transferrable Skills:*

• Certification as a Certified Alcohol and Drug Counselor (CADC) I, II, III

• Knowledge of mental health, substance abuse, legal, medical, social, and/or housing resources in the community

• Experience working in a primary care or behavioral health care setting

• Experience using electronic health record systems (Epic preferred)

TYPICAL TASKS

Client Coordination & Engagement

• Build rapport with clients and act as the point of contact within the integrated care team

• Provide client outreach to support treatment engagement (including encounter-based outreach)

• Assist clients and providers with referrals to higher levels of care

• Serve as the care team liaison with primary care

• Facilitate communication between peers, case managers, and clinical staff

Training & Team Leadership

• Lead daily interdisciplinary huddles to ensure coordinated, client-centered care

Administrative & System Support

• Manage in-basket messaging within electronic health record systems (e.g., Epic)

• Support scheduling of client appointments and care team tasks

• Assist with feedback letters and crisis line coordination

• Audit documentation, service plans, assessments, and program compliance

• Track service conclusions and ensure completion of required service plans

Resource & Referral Management

• Generate, monitor, and complete referrals to internal and external services (e.g., medical, behavioral health, SUD, PCP, peer support, housing, and other specialty services)

• Collaborate with case managers on linkage to outside providers and community support

Project & System Development

• Support service coordination for hospital step-downs and emergency department discharges

• Manage intake scheduling and team coverage needs

• Coordinate with COD and other SHIFT partners to ensure continuity of care

Additional Support Functions

• Coordinate new client onboarding and related Access processes

• Facilitate medical-only transfers and UA coordination

• Participate in program compliance reviews and audit preparations

WORK SCHEDULE

This position works 40 hours during a standard workweek of Monday through Friday or alternate workweek of Monday through Thursday, or Tuesday through Friday.

Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
